How Your Assessment is Conducted

How the ADHD Assessment Process Works
Step 1: Initial Appointment
Book an initial consultation with Dr. Gavin Lobo to discuss your symptoms and concerns. If adult ADHD is suspected, our Registered Nurse will email you the required diagnostic questionnaires and intake forms.
Step 2: ADHD Questionnaires
Several standardized questionnaires must be completed before your clinical assessment, including:
- ADHD lifestyle and personal history questionnaire.
- Mood and anxiety screening forms.
- AUDIT-C (alcohol intake screening)
- ASRS (adult ADHD screening tool)
- WURS (childhood symptom retrospective questionnaire)
Step 3: Registered Nurse Screening Appointment
Once your completed questionnaires are returned, a Registered Nurse screening appointment will be arranged. This crucial safety check includes:
- Blood pressure and heart rate monitoring.
- Height and weight checks.
- A brief Mental State Examination.
- Laboratory blood test forms.
- An ECG (electrocardiogram) for patients aged 45 years and over.
Step 4: Comprehensive ADHD Assessment
After your questionnaires and laboratory tests are processed, you will meet with Dr. Gavin Lobo for a comprehensive clinical assessment. This appointment includes:
- A structured clinical diagnostic interview.
- An in-depth review of your personal history and symptoms.
- A formal DSM-5 diagnostic assessment.
- A collaborative discussion of your ongoing support and treatment options.
Step 5: Follow-Up Review
If medication is prescribed, a mandatory review appointment is required 4 weeks after starting treatment to monitor your progress, check vitals, and ensure ongoing management is safe and effective.
