To enrol with us you can come and talk to us at reception, or fill out our enrolment form by downloading it below and fill it out, sign it, scan it and email it back to reception@gavinlobohealth.co.nz. You can also fill out the patient questionnaire, which you can download below.
We will need the original paperwork brought in along with a copy of each family member’s Passport or Birth Certificate, and any visa's you hold, e.g. 2+ year work visa or resident visa.
